• Photo Identification: This can be a State ID, Driver’s License, Passport or other form of government issued ID.
• Bloodborne Pathogen Certification:  This certificate must be current at the time of the board certification exam and come from an approved issuer for your geographical location.  
• Primary Training Certificate: This must be a 100-hour Micropigmentation Fundamentals Training Certification from an approved AAM or SPCP instructor.  The course content must include brows, eyeliner, and lip procedures using a tattoo machine.  The course should also include 6 procedures (2 Eyebrows + 2 Eyeliners + 2 Lips) on live models that count toward your total apprenticeship requirement. 
• Letter of Apprenticeship:  This letter must be provided by a board certified practitioner.  The total number of required supervised apprenticeships is 15 procedures (5 Eyebrows + 5 Eyeliners + 5 Lips).  Procedures completed during primary training may be submitted as part of this requirement.  Eyebrow procedures can include a combination of machine and microbladed eyebrows.  At least two sets must be machine work and up to three sets can be microbladed.  The Apprentice must do left and right eyebrow on the same model to qualify.  Eyeliner procedures may include top, bottom, or both.  If both top and bottom of the same model is included, it counts as a single procedure.  The Apprentice must do left and right eyebrow on the same model to qualify.  Lip procedures may be either a liner or full lip.  If lip liner and lip shading are done on the same model, it counts as a single procedure.  The Apprentice must do top and bottom lip to qualify.  
• Apprenticeship Photos: Photos of each apprenticeship procedure must show clearly demonstrate the entire procedure area.  Photos that show only a profile view will not be accepted.  Two photos are required for each apprenticeship procedure.  The first is a “before” photo and the second is an “after” photo as shown in the table below:      
• Liability Insurance: The policy must include General Liability and Professional Liability with coverage of at least $1M aggregate/$1M per occurrence/$1M products completed.  Proof of coverage can be shown with a policy declaration page which shows the name of the person being insured, insurance company contact information, amount of payment, amount of coverage, dates of coverage, and coverage minimums.
